Michigan Nose & Sinus Health strives to provide the highest level of service to our patients. To help make your first visit to our practice as smooth as possible, please read the information for Our Patients. You’ll find all the practical information you need, such as a map and directions to our offices, practice hours, payment policies and more. We also describe our first visit procedures.
Please bring the following to your appointment:
- Insurance card (s)
- Photo ID
- A list of current medications
- Referral, if required by insurance
What to expect on your first visit
To save time at your appointment, please fill out one of the on line New Patient Forms.
If you choose not to complete one of the online forms, please arrive 15 minutes prior to your appointment so you can complete the necessary paperwork.
Online Patient Forms
You can fill out an online form and register your request whenever you need it. Just click on the button below and page with the opportunity to fill will be opened.
Billing and Insurance
Some healthcare insurance plans require you to have a referral from your primary care physician prior to seeing a specialist. Also, many plans require preauthorization prior to surgeries or any surgical procedures. Our office will assist in initiating any referral of preauthorization; however, it is the patient’s responsibility for any insurance denying payments for our services.
We participate with most health insurance companies. For a comprehensive list of insurances or to see whether or not we participate with your insurance company, please contact our office at (810) 695-3766.

The fact sheets are approved by the American Academy of Otolarynology. We want you to be well informed of what treatments and procedures that are available in order to provide you the best possible care.
